Writing a Demand Letter

Hello again,

I need some ideas for writing up a demand letter for unpaid child care. I want to send an formal letter, before filing a contempt of court. Anyone have ideas on a good layout? What it should include, etc.?

List
 
Attach proof
 
use please, request, NOT DEMAND
 
send via traceable mail -- I prefer priority confirmed delivery
 
and then wait
 
then follow through with the court.

and add one sentence quoting where in the court order it states she is required to pay. "according to the court document dated xxx, paragraph xx"
